Head Teacher Declaration Form

Biology A / Biology B / Chemistry / Physics Non-Exam Assessment (NEA)

The assessment of practical skills is a compulsory requirement of the course of study for this specification. All students must carry out a minimum of 12 practical activities which meet the requirements of Appendices 5b ('Practical skills identified for direct assessment and developed through teaching and learning') and 5c ('Use of apparatus and teaching') from the prescribed subject content.

If your centre has entered student(s) for this qualification, then your head teacher must sign the declaration below.
Head teacher Declaration
I declare that reasonable steps have been taken to ensure that each student has completed the
minimum of 12 practical activities as set out above in accordance with the requirements of this
specification.

I confirm that:

i. Each student has made and kept a contemporaneous record of their work and the knowledge,
skills and understanding that they have gained from completion of the practical activities.

ii. Students have consistently and routinely demonstrated the required standards across all
the common practical assessment criteria (CPAC).

iii. Students have been assessed against the CPAC.

iv. Students have been judged competent with respect to the requirements in the Appendices
5(b) and 5(c) of this specification.

v. Only students who have demonstrated the required standard across all the requirements of
the CPAC have been awarded a pass for the practical endorsement component.

vi. I certify that to the best of my knowledge, the evidence submitted for assessment is the
candidates own. The candidate has clearly referenced any sources and any artificial
intelligence (AI) tools used in the work. I have not solely used AI to assess the learner’s
work. I understand that false declaration is a form of malpractice.
