Find out how to request an official document following a change or amendment to your name. Name changes or amendments can be made following gender reassignment and we will also consider other name change and amendment requests.
Where a name change or amendment is not required
If you need to share your certificate with a third party but your current name no longer matches the name on your certificate, the third party will normally accept the document if you share it with the official documentation which confirms your name change, for example, a deed poll or marriage certificate. If you have lost your certificate, you can order a new one in your old name (using our replacement documents service) which you can then share alongside the official documentation which confirms your name change.
Where a name change or amendment is required
We understand there may be circumstances where you need to contact us to request a change to your certificate(s).
Please select the relevant replacement certificate service on our online form, explain the circumstances in the additional information field and attach the evidence listed below from the email you’ll receive once you’ve completed your application.
A name change isn’t guaranteed but we will then confirm whether or not we need further evidence or if the change can be made or not. If we can make the change, we will ask you to return your original certificates to us before we send you your official document. If we can’t make the change, we will issue a full refund.
Information we require for a name change or amendment
Please attach the following information to your initial application:
- Two forms of identification in your new or correct name, one of which confirms your current address, for example, a copy of your passport, photocard driving license or birth certificate and a copy of a recent bank statement or utility bill (no more than 3 months old containing your current name and address)
- Evidence which links the name on your certificate to your new or correct name, for example, a letter from the centre which references the old name or confirms it was spelt incorrectly, or proof of your change of name, for example, a copy of your deed poll.
Please note we only require copies of these documents and not originals.
If our priority service is selected (only available for certain qualifications), we will be able to provide your document from 5 working days after we have received the necessary evidence.

Amendment following gender reassignment
If you need to amend your certificates following gender reassignment, please send an email to firstname.lastname@example.org with the following evidence:
- 2 forms of identification in your new name, 1 of which confirms your current address; for example, a copy of either your current passport, photocard driving licence or birth certificate, together with a copy of a recent (no more than 3 months old) bank statement or utility bill; and
proof of your change of name, for example a copy of your deed poll, statutory declaration or similar official documentation (showing both your previous and current names).
We will also ask you to return your original certificates to us.
The email@example.com email address is only used for gender reassignment cases, and any other emails sent to this address will not receive a response.
